As of this morning, Lastpass has pushed a server-side fix to resolves the issue with Duo-protected authentications. As the fix is server-side, no further actions, such as updating the LastPass extension, are required.

LastPass has released update 4.1.39 to their Chrome extension that resolves this issue with the Duo Authentication Prompt. Our Operations Team has confirmed the service degradation leading to authentication slowness or failure has been fully resolved as of 02:35 UTC (10:35 p.m. ET). The incident timeline occurred as follows: 01:49 UTC: Authentication latency begins to grow. 02:05 UTC: Authentication latency reaches significant levels, resulting in increased timeouts or failures. 02:26 UTC: Authentication latency begins returning to normal levels. 02:30 UTC: Service restored for users who begin authentications now; some very slow authentications still in progress. 02:35 UTC: Service is fully restored.

DUO23: Authentication issue
From approximately 21:24 to 21:40 UTC, all authentications to the Duo deployment DUO23 failed. Our operations team acted quickly to resolve the issues and authentications are currently completing as normal. This only affected authentications to the DUO23 deployment, and other deployments were unaffected. Our operations team is investigating the root cause of the issue and are taking measures to ensure it won't happen again.

Authentication issues for some DUO1 customers
From approximately 9:56 PM to 10:06 PM UTC, due to an incorrectly applied DNS change made to the duosecurity.com zone, some customers hosted on DUO1 may have been unable to contact their Duo admin panel, API endpoints, or other aspects of the service between these times. Our operations team has acted quickly to resolve the issue, and customers have confirmed successful resolution of their API hostname addresses, as well as successful authentication. If you're still seeing an issue, please try flushing your DNS cache and try again.

Push Delivery Issues for Android devices
Between approximately 9:45PM and 10:15PM UTC today, about 25% of Android device users may have had a problem receiving push notifications. This is due to an issue with the Google Cloud Messaging service, related to the incident that happened on 4/19 that we posted about. Google has been aware of the issue since 4/19 and are investigating. Currently, push notifications are being received as normal. During these types of issues, checking for authentication requests within the Duo Mobile app is expected to function as normal.

Push Delivery Issues for Android devices
From approximately 19:35 to 19:55 PM UTC, a small number of our customers experienced issues receiving push notifications on Android phones. This affected approximately 3% of push notifications sent to Android devices across all customer deployments. These issues appear to have subsided, and we’re still investigating root cause. It's currently suspected that the Google Cloud Messaging service was having an issue during this time.

Between 22:00 and 00:11 UTC, a portion of users experienced intermittent issues with web-based authentications. This only affected accounts that were opted-out of our new Authentication Prompt User Interface that was rolled out in early December. Client Application-based Cisco and Juniper VPN authentication issues
