DUO40: Some authentications impacted during load balancer failover

From 11:53 am. ET to 12:03pm ET, our services experienced an issue during a load balancer failover. This issue caused some user authentications to applications and some logins to the Duo Admin Panel to fail.

This issue only impacted the DUO40 deployment.

All services are now operational. We will provide an RCA as soon as it is available.
